I'm finished, but I don't see the Submit button.
Submit should show in a yellow button on the left side of your form. If that yellow button says anything other than Submit, it means you are not done with this form. There is at least one required field (outlined in red) that you have not filled in.
First, please look the form over to see if there are any red fields or buttons that you have not completed.
Second, if you have completed them all, please make sure you completed them in the correct format. For example, a date field may require you to use YYYY-MM-DD format or a Social Security Field may require ###-##-#### format.
How do I put my signature on the form?
If a signature is required, you will see a yellow box that says Sign Here on the signature line. Click the Sign Here button. In the signature box that appears, please
type in your full name and your initials, and then
click and hold your mouse while drawing your signature in the open white box that says where it says “Draw Signature.”
When you've finished drawing your signature, press the yellow Apply Signature button. You should see your signature placed on the form.
There's a required field that doesn't apply to me.
Please enter “N/A” if there is a required field that does not apply to you. All required fields (outlined in red) are mandatory, and you will not be able to submit the form without putting something in all required fields, so please simply enter “N/A” or some other text to note that that field does not apply to you.
I don't know how to answer one of the questions on this form.
If you are confused about the content of the form, please contact the person who assigned you the form. Remember, once you submit a form, your answers are final and can not be changed, so if you aren't sure what to put for a particular answer, please do not submit the form with a blank answer or with incorrect information, or else you will have to do the entire form over again.
What happens after I submit my form?
At the top of your form, you will see a table showing you the workflow of your form. You may be the only person that needs to fill this copy of the form out, or there may be other people that the form went to before you or will go to after you.
If you are the last person in this form's workflow, then once you submit the form, the person who originally assigned it to you will be notified, and this form will be marked as complete.
If you are not the last person in the workflow, when you submit, whoever is listed immediately after you in the workflow will receive an email with a link to the form so that they can fill out their part. Each person involved in the workflow will be emailed step-by-step until each person has submitted their portion of the form.
Can I make changes to my form?
While you have the form open, you can make any changes you'd like. You can also click Save and Finish Later under the Options tab. This will allow you to come back to this form (by clicking the same link you originally clicked to open it). When you come back to a saved form, all the responses you've entered will still be there.
Once you press Submit, though, you will not be able to make any changes. So, please be sure that before submitting, you review all your responses and that all of your answers are final.
Can I save my form and finish it later?
Under the Options button at the top of your form, you will see a button to Save and Finish Later. You can click this anytime to save the answers you've worked on up to that point. When you re-open the form (using the same link you clicked on to access it initially), all of your responses should still appear.
The form is asking me for information I don't think I'm supposed to be filling out.
You can enter “N/A” in fields that don't apply to you, but if it looks like the form was sent to the wrong person or if you see fields that you know someone else in the form workflow (shown in the information box at the top of your form) should be completing, please contact the person who assigned you this form as soon as possible.

Can I print my form out and turn it in on paper or fax it?
Your form assigner is expecting you to complete the online copy of this form. When you turn in a form on paper, the form assigner or someone at the collecting institution will have to file the form and do data entry to compile your response, whereas when you complete it electronically, all your responses are automatically compiled and organized without any manual effort. (Plus, it saves paper!)
Will I have access to my form once I've finished it?
Absolutely. If you are someone who has a Teamworks user name and password, you can log into Teamworks anytime to see your submitted forms. When you log in, under the Home tab, click on Forms, and then click the link in the upper right that says “Show My Completed Forms.” If you've completed your part of this form, but the form still needs to be filled out by others, it will show up under Forms Waiting on Others.
If you accessed this form by clicking a link that was emailed to you, when the form is completed, you are going to get an automatic email with the final copy of your form attached as a PDF for your records.
How do I attach a file to my form?
If you are required to attach a file to this form (such as a drivers license copy, insurance card, transcript, or picture), you will see a yellow button that says Attach File. Please click on that button.
In the attachment pop-ups, there will be two options: “Send by Fax” or “Choose File.” DO NOT CHOOSE SEND BY FAX. Click Choose File.
Select your file just like you would if you were selecting an email attachment. You will be asked, “Would you like to add another attachment to this document.” Press YES until you have added all the documents you need, and then press NO. Note: Once you press NO, you can not change your document or add another, so please make sure you have selected the correct document.
